[MTOS-dev] (More) MT4 keyboard shortcuts

Following from this:

I'm proposing the attached image as an expansion.

Everything except the drop-downs has been implemented in the attached .tmpl
file, if anyone wants to give them a try as an alt template for comfort.
Just place in alt-tmpl/cms/include
I can generate a diff for submission after anyone who might care comments.

The problem I've run into is that the drop-down items don't exist in the app
templates but are registered in CMS.pm and then generated. I'm not sure how,
if possible, to get around this and suspect it would involve actually
modifying the CMS.pm definitions and possibly even adding an entirely new
accesskey attribute to that. All of which is beyond me.
I think I'd also ideally like to see the behavior change so that these
potential accesskeys trigger the drop-down rather than default to the first
item below(which is a bit odd), but that may not be possible.

Thoughts?
